Obesity
It has been said, "Your body is a temple." Few people realize this and even fewer treat it as such. Your body is the only material thing that you cannot lose until death. Everything else can be taken away. Your body is all you have in the world.
We are primarily responsible for our health. There are the risk factors of genetics, toxic environments and the like, but our emotional and lifestyle choices determine our health and well being more than anything else.
Over the recent year rates of overweight and obesity are escalating rapidly to epidemic proportion in many parts of the world including India, reflecting increasing consumption of high energy diets rich in fats and sugars, compounded by declining levels of physical activity.
Define:
Obesity is a state in which there is a generalized accumulation of excess adipose tissue in the body leading to more than 20 percent of the desirable weight.
How to assess obesity?
There are several methods used world wide to establish a correct weight, over weight and obesity. Such methods include.
Body mass index
Height weight range
Brocas Index
Body fat measurement
Others
1. Body mass index in this method the weight in Kgs is divided by the height in Mtrs Square.
BMI = Weight in kilogram
Height in (meter) 2
Experts from different scientific bodies including WHO have recommended obesity cut-off for Asians to be as follows:

Grading of BMI (kg/m2) is as follows.

Not obese < 25 and below

Overweight 23 – 25

Obese > 25

Grade I obesity 25 – 29.9

Grade II obesity 30 - 40

Grade III obesity 40 > and above.

2. Brocas Index The formula for Brocas index is, height in centimeters minus 100 is equal to ideal weight in kgs. For example if an individual’s height is 155cms or (1.55 Mtrs), then minimum 100 from the height and in case the weight is in excess of 55 kgs, the person is considered overweight. Thereafter 20% above ideal weight is considered obese.
3.  Body fat measurement:
1. Waist hip ratio- Most people store their body fat in two distinct ways:
a Around middle (apple shape)
b Around hips (pears shape)
Being apple shape (more fat around the waist) places them in higher risk category than the pear shaped (carrying more fat on hips).
The measure of the two is called waist hip ratio (WHR)
  WHR (men) = 0.89
  WHR (women) = 0.81
Waist measure below the rib cage above the naval.
Hip measure at its maximum.
1. Waist circumference - is another quick measure to determine obesity especially in Indian population, due to more marked central obesity.

Normal Range

Men: 85 cm

Women: 80 cm
4. Height weights range The simplest and wildly used method is to compare one's height and weight against given Standard heights and weights chart.

5.other simple methods include for example THE MIRROR TEST. In this you may simple stand nude in front of a mirror to self assess, whether or not you are fat. In THE BELT TEST, you simply measure your stomach at the navel, and if it is larger than your upper chest measurement (above the breast in ladies) then you are over fat. In THE PINCH TEST you simply pinch the roll of flesh in the abdomen region and in case it is more than I inch thick, it is indicative of being over fat.

Etiology / what causes obesity?
Usually obesity is due to positive energy balance. That is, the intake of calories is more than the expenditure of calories.
Genetic factors
Poor eating habits
Physical inactivity
Endocrine factor (Hypothyroidism, hypogonadism, Cushing’s syndrome)
Trauma (due to damage to the hypothalamus after head injury)
Prosperity
Psychological factors (food is used to relieve boredom, reduce anxiety, or alleviate stress)
How to control?
Long-term weight control requires a permanent sense of awareness and lifelong habit change. Clients must be willing to devote much time and long-term effort in order to change lifestyle habits that have contributed to the weight problem.
Are crash diets are good?

Crash diets, gimmicky fads and schemes may cause temporary weight loss due to an increase in eating awareness and a forced change in habits, yet any results are impermanent. As soon as diet is forsaken, any lose weight is regained- usually with additional poundage.

Weight reducing diets must be low in total calories and yet ensure optimum intake of all nutrients. Diet restriction should be combined with exercise.

What is the goal of weight loss program?
In weight loss programs, individual eating practices and levels of physical activity function together to determine body weight and body fat. The ultimate goal is to establish a permanent lifestyle changes in both food consumption and physical activity patterns for the attainment and maintenance of healthful body weight and fat.
Why Lifestyle changes are needed?

Focus on health, not diets. Diets are hard to maintain and usually do not work in the long run. It is very hard to stick with a diet that includes lots of big changes in your eating habits.

Instead of a diet, focus on lifestyle changes that will improve your health and achieve the right balance of energy and calories. To lose weight, you need to burn more calories than you take in. You can do it by eating healthy foods in reasonable amounts and becoming more active. And you need to do it every day.

Little steps mean a lot. Losing just 10 pounds can make a difference in your health.

When one should take medicines or have surgery?

Surgery and medicines do not work by themselves. Most people also need to make changes in what they eat and how active they are.

Before your doctor will prescribe medicines or surgery, he or she will probably want you to work on diet and activity for at least 6 months. Even if your doctor gives you medicines or recommends surgery, you will need to stick with your new healthy habits for the rest of your life.
